Consent Form For the Use and Processing of Personal Data

Merit Medical’s organization uses and processes personal data in various ways, for various reasons. These are:

Congresses and Conventions

  • Personal data collected from participants: Full Name, hospital details (including hospital address and department name), job title, email address, mobile phone number, professional address, personal address, field of interest/expertise, Merit products. In addition photographic, film and video material may be collected during the attendance of congresses and conventions.
  • The Marketing and Training departments collect data and share with Regional VP. Regional VP shares data with various Sales Representatives, which could be a Merit employee, distributor or agent, depending on the region. The Sales Representative may contact the participant.
  • The Marketing and Training departments keep the above described data on file for future emailing lists for the benefit of e-blasts, emails and other mailings related to the participants subject of interest, related to product selection and related to the congress subject, or for the purpose of inviting the participant to events and (training) courses.
  • The Marketing and Training departments may share the participant’s personal data with hotels and/or taxi companies, if and when such third party services are required.
  • The Marketing and Training departments may use and/or transfer, and/or process any photographic, film and video material of the participant collected during the congress or convention to advertise on various media.

Training courses

  • Personal data collected from attendees and/or lecturers: Full Name, hospital details (including hospital address and department name), job title, email address, mobile phone number, professional address, personal address, passport details, field of expertise, Merit products. In addition photographic, film and video material may be collected during the attendance training courses.
  • Marketing and Training departments collect data and share with Regional VP. Regional VP shares data with various Sales Representatives, which could be a Merit employee, distributor or agent, depending on the region. The Sales Representative may contact the participant.
  • Mobile phone numbers will be shared with Regional VP. Regional VP will provide such phone numbers to the correct and relevant Sales Representative, which could be a Merit employee, distributor or agent, depending on the region. The Sales Representative may contact the participant.
  • The Marketing and Training departments keep the above described data on file for future emailing lists for the benefit of e-blasts, emails and other mailings related to the participants subject of interest, related to product selection and related to the congress subject, or for the purpose of inviting the attendee to (other) events and (training) courses.
  • The Marketing and Training departments may share the attendee’s personal data with hotels and/or taxi companies, if and when such third party services are required.
  • The Marketing and Training departments may use and/or transfer, and/or process any photographic, film and video material of the attendee collected during the training course to advertise on various media.

Merit Medical and affiliate Websites

Merit Medical receives information and personal data from persons who have used its web registration system. Such information and personal data is transferred to the relevant department for follow up. Such data may also be kept/saved by Merit Medical’s Marketing and Training departments to use for future communications (emails, mailings, e-blasts, events and training course invitations).

Personal contact

Merit Medical may transfer personally received emails internally to the Marketing, Training and/or Customer Service departments  or even to other related departments. Any personal data contained within or following from such an email may be used for follow up and data may be kept by said departments for future communications (emails, mailings, e-blasts, events and training course invitations)

The [participant/attendee/lecturer/consultant/applicant] acknowledges that his or her personal data, including but not limited to their name, (work)address, date of birth, passport telephone number and email address, as submitted by the [participant/attendee/lecturer/consultant/applicant] is processed by Merit Medical in the above described manner.

The [participant/attendee/lecturer/consultant/applicant] herewith gives his or her explicit consent that Merit may use and process the above listed data for the above described purposes.

The [participant/attendee/lecturer/consultant/applicant] acknowledges that some of the above described personal data may be provided to persons, companies and/or institutions located in countries outside of the European Union, which may provide for a lower level of data protection and gives his or her explicit consent to a transfer of the above data to such persons, companies and/or institutions located outside the European Union.

The [participant/attendee/lecturer/consultant/applicant] agrees that Merit may contact him or her by email for the purpose of providing the [participant/attendee/lecturer/consultant/applicant] with marketing and information material.

The [participant/attendee/lecturer/consultant/applicant] gives his or her explicit consent that Merit may use his or her personal data for its own marketing purposes but also for third parties’ marketing purposes, and that they may be contacted by Merit for these purposes by email. Merit will in this context not provide any personal data to third parties.

The [participant/attendee/lecturer/consultant/applicant] acknowledges that the above described consent can be withdrawn any time (without giving reason) by sending an email to [email address].
